Acclaim Health is a registered charity providing extraordinary community care and support in Halton Region. Our goal is to provide the care people need to live well at home for as long as possible. Last year we cared for almost 25,000 clients in Halton, two thirds of whom were over the age of 65. Our team of over 600 staff members and over 650 volunteers is committed to high-quality care, reflecting our 97% client satisfaction rate.
For isolated older adults, we offer Friendly Visiting, Tele-Touch (reassurance phone calls), the Home Support Exercise Program (to improve strength, endurance and balance) and our Companion program, which fills the gap between what volunteers can provide and what the health care system can offer in terms of practical assistance in the home and with errands.
Our dementia care services consist of day programs for adults with dementia or related diseases in Oakville and Burlington, caregiver support and education, and a Memory Visiting program provided by volunteers for those in the early stages of the disease.
Our end of life care includes Hospice Care for those choosing to die at home, bereavement support for families, and palliative care consultation for medical professionals to improve their knowledge and skill in hospice palliative care.
Our team of nurses and personal support workers provide the medical and personal care people need to maintain their physical health, including being the sole provider of home palliative care in Milton, Oakville and Halton Hills.
We have achieved exemplary standing – the highest possible – with Accreditation Canada and are accredited with the Imagine Canada Standards program.
Our Impact
No one wants to be in the hospital or go to a long-term care home unless it’s absolutely necessary. Acclaim Health provides families with a wide range of services so that people can live in their own home for as long as possible.
